Five Step Process

The Five Step Process describes the care management process as it relates to assessment, planning, facilitation, care coordination, and evaluation/advocation. This module will focus on each step and the care manager's role with each.

The expected time to complete this module is 60 minutes.

CE Requirements: CE participants who successfully view both lessons of the module and complete the online CE request process including required evaluation with email address will earn 1.0 Nursing or Social Work CE contact hours

A Certificate of Completion is available for non-CE participants

Lesson 1: Five Step Process

Objectives:

  • Describe the goals of care management
  • Define the care management five step process
  •  the care manager's role in each step of the Five Step Process

Objective:

  • Apply learning from the Five Step Process to a case study
