Introduction to Advance Care Planning and Palliative Care

Introduction to Advance Care Planning and Palliative Care describes the advance care planning process, how to approach the discussion, and document types. Care mangers will be exposed to the fundamentals of palliative care, including facilitating the discussion, and defining the difference between palliative care and hospice.

The expected time to complete this module is 60 minutes.

CE Requirements: CE participants who successfully view all three lessons of the module and complete the online CE request process including required evaluation with email address will earn 1.0 Nursing or Social Work CE contact hours

A Certificate of Completion is available for non-CE participants

Objectives:

  • Define Advance Care Planning
  • Describe the types of Advance Care Planning documents
  • Describe the barriers to Advance Care Planning
  • Describe the elements of the advance care planning discussion

  • Define palliative care and the core elements
  • Identify patients who would benefit from palliative care services
  • Communicate effectively with patients facing serious illness regarding palliative care services
  • Compare/contrast hospice and palliative care
