Medication Management

Medication management facilitates the appropriate understanding and use of medications in order to control illness and promote health. Medications are utilized in 80 percent of all treatments and impact every aspect of a patient's life. Medication errors harm an estimated 1.5 million people each year, costing at least $3.5 billion annually. A third occurs on discharge from the hospital with some ending in a medication adverse event within three weeks of discharge. In many of these cases patients do not have an accurate list of their medications and do not understand the purpose of their medications, how or when to take them or understand the directions on a prescription drug label.
