Comprehensive Primary Care Plus (CPC+)

The Comprehensive Primary Care Plus (CPC+) program is a Centers for Medicare & Medicaid Services (CMS) five-year (2017 to 2021) payment and care delivery model.  Over 440 primary care practices in Michigan were selected to participate in the program.  As per CMS, no additional practices will be added  to the demonstration.   CMS maintains a portal for practice reporting and a national CPC+Connect site for users authorized by CMS within CPC+ practices. 

In addition, three commercial payers (Blue Cross and Blue Shield of Michigan, Priority Health, and HealthScope Benefits)  were selected by CMS to participate as commercial payers in Michigan.  These commercial payers work together to jointly support multipayer coordination and provide resources and tools that can be easily accessed by the CPC+ community in Michigan.  These resources are housed on the Michigan Multipayer CPC+ website at www.cpcplusmichigan.org.  This site requires no password or user authorization so that information can be easily accessed by CPC+ practices, POs an other partners in Michigan.   It is a Michigan-specific site maintained by the multipayer support team at the University of Michigan on behalf of the participating commercial Michigan payers.
