MICMT Self Managment Support Course Statewide Trainer Organization Application and Resources

Steps to complete the MICMT SMS Course Statewide Trainer Application:

Step 1 - Review the following:

  1. MICMT Self-Management Support (SMS) Course Trainer Application Overview. Click Here
  2. The MICMT SMS Trainer Organization Requirements and Lead Trainer Qualifications document. Click Here
  3. The MICMT Standardized SMS course curriculum overview. Click Here
  4. MICMT SMS Standardized course materials:

Step 2 - Complete the MICMT SMS Trainer Application:

  1. Complete the Trainer Organization SMS curriculum template. Click Here
    • Purpose of the template: a) inform MICMT if your course will include content in addiiton to the MICMT SMS standardized curriculum, b) describe the content addition and c) indicate for each agenda topic the modality you will use. (n/a should be used if not adding content)
    • We encourage you to personalize the MICMT Standardized SMS course content by including content which is specific to your organization.
      • As you continue to complete the web based MICMT Statewide SMS course Trainer Application, you will receive a prompt to upload the Trainer organization SMS curriculum template
  2. Complete the web based MICMT Statewide SMS course Trainer Application Click Here

Step 3 - MICMT leadership team notification of application approval status:
