September 2019, MICMT launched a standardized Self-Management Support course curriculum and a Statewide Trainer organization application for this course. The intent is to create a standardized Self-Management Support training experience across the state. The MICMT SMS standardized course is now offered by the MICMT team and MICMT Self-Mangement Support Course Trainer Organizations.

The MICMT Self-Management Support course is designed to prepare the healthcare professional in providing self management support to patients in the primary care and specialty care physician offices.
Course content is applicable to all Care Managers and healthcare professionals in the ambulatory care setting, working with patients. The MICMT Self-Management Support curriculum prepares healthcare professionals on how to talk with patients about their health using motivational interviewing as they assist patients with health related goals.
MICMT requires the PO leader, practice manager or attendee's direct manager to register the care manager for the Self-Management Support course. This will facilitate accuracy of completion of the course registration fields and access to longitudinal resources for your staff.
MICMT SMS Course
MICMT SMS course is a blended learning activity and consists of the following components:
- Recorded webinar, orientation to MICMT SMS course
- Self-study web based modules
- In-person training, one 8 hour day
- MICMT SMS Course Statewide Post Test and course evaluation
- 30 minute Virtual Practice Session with MICMT SMS Trainer
Successful course completion: Learner completes all course components as listed above, achieve a "Pass" score of 80% or greater on the MICMT SMS Course Statewide Post Test and completes a 30 minute Virtual Practice Session with an MICMT SMS Trainer. NOTE: test scores will be made available to the Physician Organizations
