Advance Care Planning

(Updated February 6th 2017)

Advance care planning is centered on individuals making their healthcare wishes known ahead of time to their family and physician, putting those decisions in writing and designating a spokesperson to communicate those wishes when they are unable to speak for themselves. No matter what age, advance care planning can give piece of mind not only to the individual but to their friends and family as well. Advance care planning often involves discussions between the patient, care manager, physician and family.
