Provider-Delivered Care Management (PDCM) Online Course

Welcome to the Michigan Care Management Resource Center PDCM Online Course. The PDCM Online Course addresses the key functions of successful care management programs and models of care for ambulatory practices. This course is available on line and may be accessed at a time that works for you.

 

Who should take this course: 

Training requirements for Lead care managers and Qualified Health Professionals are defined by BCBSM based on the program your practice has selected.

The training requirements for PDCM-Primary Care and PDCM- Specialists are summarized Here.

 

Lead care managers: defined as an RN, NP, PA, or LMSW.

Qualified health professionals:  defined as a Clinical pharmacist, LPN, Certified diabetes educator, Registered dietitian, Master of Science-trained nutritionist, Respiratory therapist, Certified asthma educator, Certified health educator specialist (bachelor’s degree or higher in health education), Licensed professional counselor, Licensed mental health counselor, or Licensed bachelors-level social worker.

What: The PDCM Online course consists of seven self-study lessons to be completed at your own pace.  Each lesson is a recorded webinar:

Steps to complete the PDCM Online course

  1. Please complete the seven lessons in the sequence offered below. You may complete the lessons over time, when it is convenient for you.  Please allow 20 to 30 minutes for each lesson.
  2. To access each lesson please click on the recorded webinar link and listen to the entire lesson.
  3. Successful PDCM Online Course completion includes:  listening to the 7 lessons, completing an online evaluation and requesting a certificate of course completion or CE.  Once you have completed the 7 lessons click this link to request CE/certificate and complete the course evaluation.
